I like it, but I would probably change the background above the line. Since it is the same shade of green, and pattern, as the top of the book, it makes the book blend in a little.
I'm not aware of the circumstances of the book, but it isn't just her child. It took two. They both should have a say about the baby, unless it was a rape and she isn't planning to abort. I'm very adamant about that issue, otherwise the woman gets to abort if she chooses, and he's left behind with an empty hand/heart.
So I'd change the caption as well as tone down the background.
